What Is a Cough in Children?

A cough in children is a natural protective reflex that allows the airways to clear themselves of secretions, viruses, irritating particles, or foreign bodies. It is very common, often accompanies most winter viral infections, and is usually benign.

It usually lasts 1 to 3 weeks with a simple viral infection. Identifying the type of cough and its context guides treatment: not all types are treated the same way. A cough that persists for more than 3 weeks or is accompanied by warning signs requires medical attention. Additional resources are available for children’s colds and coughs in general.

What types of coughs should you be aware of?

Different types of coughs point to different causes:

  • Dry cough: irritative, non-productive; may indicate the onset of a viral infection, an allergy, or irritation from dry air or tobacco smoke
  • Productive cough: accompanied by mucus, helps clear the bronchi during a respiratory infection
  • Nighttime cough: suggestive of asthma, gastroesophageal reflux, or postnasal drip
  • Barking cough with hoarseness and wheezing on inhalation: suggests laryngitis (croup)
  • Wheezing cough: a possible sign of bronchiolitis, asthma, or bronchitis
  • Cough lasting longer than 3 weeks: asthma, allergies, whooping cough, or reflux should be investigated

A sudden, unexplained cough in a child who is eating or playing should raise suspicion of foreign body aspiration: a medical emergency.

What are the main causes?

Several causes often occur together:

  • Viral infections: nasopharyngitis, bronchitis, bronchiolitis, flu, COVID-19, laryngitis, whooping cough
  • Allergies: pollen, dust mites, animal dander, mold, latex
  • Asthma: chronic cough, often at night and during physical exertion, sometimes accompanied by wheezing
  • Gastroesophageal reflux: nighttime cough, coughing after meals
  • Dry or polluted air, secondhand smoke
  • Inhaledforeign body (emergency)
  • Whooping cough: intense coughing fits, more severe in infants

Seasonal factors, the environment (pets, tobacco, houseplants), and a family history of atopy often guide the diagnosis. For fever in general, specific resources are available.

How can you relieve a cough on a daily basis?

Several simple steps can help soothe a child:

  • Plenty offluids: water, lukewarm herbal teas, and broths help thin secretions
  • Nasal irrigation with saline solution to reduce postnasal drip, which triggers coughing
  • Honey (strictly for children 1 year and older, never before): 1 teaspoon in the evening soothes a nighttime cough (proven effective in children)
  • Humidify the air: use a cool-mist humidifier or place a damp cloth on a radiator
  • Keepthe head slightly elevated while sleeping
  • Ventilate the room daily; avoid secondhand smoke
  • Rest and limit strenuous activities
  • Herbal syrups (thyme, marshmallow, plantain) as recommended by a pharmacist based on the child’s age

Codeine-containing cough suppressants are strictly contraindicated for children under 12 years of age. Mucolytic or expectorant syrups are not recommended for children under 2 years of age (ANSM recommendations). Aromatic essential oils (camphor, menthol, eucalyptus) applied directly to the skin should be avoided before 30 months of age. When Should You Seek Emergency Care?

Several signs warrant a prompt medical consultation or a call to 15:

  • Respiratory distress: rapid or wheezing breathing, intercostal retractions, flaring of the nostrils
  • Bluish discoloration of the lips or face (cyanosis)
  • Apnea in infants
  • Coughing after suspectedchoking (foreign object)
  • Persistent high fever above 38.5 °C lasting more than 3 days
  • Barking cough with difficulty breathing (obstructive laryngitis)
  • Refusal to drink, signs of dehydration
  • Cough lasting longer than 3 weeks
  • Intense coughing fits suggestive of whooping cough, especially in unvaccinated infants
  • Worsening of symptoms after initialimprovement (secondary infection)

The doctor will assess the situation and may prescribe treatment based on the diagnosis: a bronchodilator for asthma or bronchiolitis, a corticosteroid for laryngitis, or an antibiotic for whooping cough or pneumonia.
