1. WHAT IS ARKOGELULES MILLEPERTUIS, capsule AND WHAT IS IT USED FOR?

2. WHAT DO I HAVE TO KNOW BEFORE TAKING ARKOGELULES MILLEPERTUIS, capsule?

This medicine is not an antidepressant and is not indicated for the treatment of major depression. If your troubles persist and you feel, most of the time :

- significant suffering that disrupts your personal, social and professional life,

great sadness and/or loss of interest in activities and hobbies you used to enjoy, combined with some of the following symptoms : insomnia, memory problems, loss of energy, irritability, unusual guilt, dark thoughts, loss of zest for life, loss of appetite, "fidgeting", other appetite and sleep disorders, consult your doctor.

St. John's Wort preparations can adversely affect the effects of many drugs. If you are taking other medicines, particularly prescription medicines, do not take ARKOGELULES MILLEPERTUIS without consulting your doctor or pharmacist.

Never take ARKOGELULES MILLEPERTUIS:

- if you are under 18 years of age,

- if you are allergic (hypersensitive) to St John's Wort (Hypericum perforatum L.) or to any of the other ingredients of this medicine, as listed in section 6.

- in association with : anti-coagulants, anti-epileptics, hormonal contraceptives (estroprogestogens and progestogens), digoxin (used to treat heart disease), immunosuppressants (used in organ transplants), antiproteases (used in the treatment of HIV infection), antidepressants (used in the treatment of diabetes).infection), anti-tyrosine kinases (used in the treatment of certain cancers), irinotecan, telaprevir, theophylline and aminophylline (used in the treatment of certain cancers).aminophylline (used in the treatment of asthma and certain respiratory diseases), verapamil (used in the treatment of heart disease).

Warnings and precautions

- You should seek medical advice before starting treatment if you are taking other medicines.

- The antidepressant properties of ARKOGELULES MILLEPERTUIS usually become apparent within the first 4 weeks of treatment. If clinical symptoms persist or worsen, or if suicidal ideation appears, you should consult your doctor.

- Although St. John's wort extract has been prescribed for many years, no specific safety data are available on its use in patients with impaired liver or kidney function. Inform your doctor if you suffer from liver or kidney disease.

- Avoid prolonged exposure to the sun during treatment, due to the risk of skin reactions resembling sunburn (photosensitivity).

- If you are due to undergo surgery, inform your doctor if you are taking ARKOGELULES MILLEPERTUIS.

Children and teenagers

ARKOGELULES MILLEPERTUIS should not be used in children and adolescents under 18 years of age.

Other medicines and ARKOGELULES MILLEPERTUIS

If you are taking or have recently taken any other medicines, including medicines obtained without a prescription, talk to your doctor or pharmacist as ARKOGELULES MILLEPERTUIS may reduce the therapeutic effect of other medicines.

ARKOGELULES MILLEPERTUIS should never be taken in combination with :

- anti-coagulants,

- anti-epileptics

- contraceptives (estroprogestogens and progestogens),

- digoxin (used to treat heart disease),

- immunosuppressants (used for organ transplants),

- antiproteases (used to treat HIV infection),

- antityrosine kinases (used to treat certain cancers),

- irinotecan (used in the treatment of certain cancers),

- telaprevir (used to treat hepatitisC),

- theophylline and aminophylline (used to treat asthma and certain respiratory diseases),

- verapamil (used to treat heart disease),

Taking St. John's Wort in combination with hormonal contraceptives may result in reduced efficacy or even cancellation of the effect, with potentially serious consequences (e.g. pregnancy). In such cases, additional contraceptive methods (particularly mechanical ones such as IUDs or condoms) should be used.

- carbamazepine (used to treat epilepsy),

- benzodiazepines (used to treat occasional insomnia),

- propafenone (used to treat tachycardia),

-cyproterone (used to treat certain hormonal disorders),

-dronedarone (used to treat heart rhythm disorders),

-ivabradine (used to treat angina),

-simvastatin (used to lower blood lipid and cholesterol levels),

-telithromycin (antibiotics),

-ulipristal(emergencycontraceptive).

- Iariprizole (used in antipsychotic treatment).

ARKOGELULES MILLEPERTUIS may cause undesirable effects such as: nausea, dizziness, headache, anxiety, abdominal pain, agitation, mental confusion in association with :

- serotonin reuptake inhibitor antidepressants (sertraline, paroxetine, nefazodone),

- anti-migraine drugs of the triptan family,

- buspirone (anxiolytic).laripiprazole (antipsychotic).

If you are due to undergo surgery, please inform your doctor that you are taking.

ARKOGELULES MILLEPERTUIS. If necessary, stop taking it (risk of interaction with products used for local or general anaesthesia).

3. HOW TO TAKE ARKOGELULES MILLEPERTUIS, capsule?

Always take this medicine exactly as instructed in this leaflet or as directed by your doctor or pharmacist. Check with your doctor or pharmacist if in doubt.

Dosage

Adults :

2 capsules morning and evening.

Preferably taken at the same time each day.

Use in children and adolescents

ARKOGELULES MILLEPERTUIS is not recommended for children and adolescents under 18 years of age.

Method of administration

Take by mouth. Swallow capsule with a large glass of water.

Duration of treatment

6 weeks.

Effects generally appear within 4 weeks. If symptoms persist during treatment, consult a doctor or pharmacist.

If you have taken more ARKOGELULES MILLEPERTUIS than you should have :

Consult your doctor or pharmacist immediately.

Overdose may cause convulsions and confusion.

After ingesting large doses of this drug, protect yourself from sunlight and other sources of UV radiation for 1-2 weeks.

If you forget to take ARKOGELULES MILLEPERTUIS:

Do not take a double dose to make up for the one you forgot to take.

4. WHAT ARE THE POSSIBLE SIDE EFFECTS?

Like all medicines, this drug may cause undesirable effects, but they do not occur systematically in everyone.

Undesirable effects of varying degrees of discomfort may be expected, the frequency of which is unknown:

- gastrointestinal disorders (such as nausea, abdominal pain, diarrhea),

- allergic-type skin reactions,

- fatigue or restlessness,

- excessive sensitivity to sunlight (photosensitivity), with skin reactions resembling sunburn if exposed to the sun during treatment.

- sensitivity disorders such as tingling or burning sensations.

5. HOW TO CONSERVE ARKOGELULES MILLEPERTUIS, capsule?

Keep out of sight and reach of children.

Do not use this medicine after the expiration date indicated on the packaging after "EXP". The expiration date refers to the last day of the month.

Store at temperatures not exceeding 25°C.

Do not dispose of any medicine down the drain or in the household waste. Ask your pharmacist to remove any medicines you no longer use. These measures will help protect the environment.

Ce médicament contient un extrait de millepertuis, plante qui a été l'objet de nombreux travaux scientifiques dans le traitement des états dépressifs. Les données qui ressortent de ces études laissent penser que le millepertuis possède une activité dans le traitement des dépressions légères. Ces études ayant porté sur des prises limitées dans le temps, ce médicament ne peut être actuellement considéré comme un véritable antidépresseur et il n'est recommandé que dans le traitement des états dépressifs transitoires.
Il est traditionnellement utilisé dans le traitement des manifestations dépressives légères et transitoires.
CONTRE-INDICATIONS  (sommaire)
Ce médicament ne doit pas être associé aux anticoagulants oraux, aux contraceptifs oraux, à certains anticonvulsivants, à certains traitements anticancéreux (inhibiteurs des tyrosines kinases), à certains traitements du VIH (antiprotéases), aux immunosuppresseurs et aux médicaments contenant de la digoxine, de la théophylline, du vérapamil, de l'irinotécan ou du télaprévir.
ATTENTION  (sommaire)
La dépression est une maladie sérieuse qui justifie un avis médical lorsque l'état de tristesse n'est pas justifié par un événement majeur ou lorsqu'il se prolonge longtemps après un éventuel événement déclenchant.
Bien que ce médicament soit en vente libre et ne contienne qu'un extrait de plante, il ne devrait pas être utilisé sans avis médical en cas de traitement médicamenteux en cours. En effet, il peut être à l'origine d'interactions médicamenteuses, notamment avec les contraceptifs oraux.
L'efficacité du millepertuis ne doit pas être surestimée et la prise d'un antidépresseur «classique», prescrit par un médecin, associé à un soutien psychologique, peuvent être nécessaires en cas de persistance des symptômes.
Évitez de vous exposer au soleil pendant le traitement : risque de réaction cutanée ressemblant à des coups de soleil (rougeur de la peau, démangeaisons), même en cas d'exposition minime aux rayons ultraviolets.
INTERACTIONS MEDICAMENTEUSES  (sommaire)
Ce médicament ne doit pas être associé aux anticoagulants oraux de la famille des antivitamines K, aux contraceptifs oraux, à certains anticonvulsivants (acide valproïque, éthosuximide, felbamate, fosphénytoïne, lamotrigine, oxcarbazépine, phénobarbital, pérampanel, phénytoïne, primidone, tiagabine, topiramate, valpromide, zonisamide), aux immunosuppresseurs (ciclosporine, évérolimus, sirolimus, tacrolimus), aux antiprotéases, à certains traitements anticancéreux (inhibiteurs des tyrosines kinases) et aux médicaments contenant de la digoxine, de la théophylline, du vérapamil, de l'irinotécan, du télaprévir, du daclastavir, du dasabuvir, de l'itraconazole, du voriconazole, du ticagrélor ou de l'oxycodone : risque de diminution des concentrations de ces médicaments par le millepertuis avec baisse de leur efficacité.
Il peut également interagir avec de nombreux autres médicaments. Signalez à votre médecin ou à votre pharmacien tout autre traitement en cours.
GROSSESSE et ALLAITEMENT (sommaire)
L'effet de ce médicament pendant la grossesse ou l'allaitement est mal connu : son utilisation est déconseillée chez la femme enceinte ou qui allaite et chez la femme en âge de procréer sans contraception efficace.
MODE D'EMPLOI ET POSOLOGIE  (sommaire)
Les gélules doivent être avalées avec un grand verre d'eau, de préférence à heure régulière.

Posologie usuelle:

  • Adulte de plus de 18 ans : 2 gélules matin et soir.
La durée du traitement ne doit pas dépasser 6 semaines sans avis médical.

anticoagulants
Médicament qui empêche le sang de coaguler et qui prévient donc la formation de caillots dans les vaisseaux sanguins.
Les anticoagulants sont utilisés pour traiter ou prévenir les phlébites, les embolies pulmonaires, certains infarctus. Ils permettent aussi d'empêcher la formation de caillots dans le cœur lors de troubles du rythme comme la fibrillation auriculaire ou en cas de valve cardiaque artificielle.
Il existe deux grand types d'anticoagulants :
  • les anticoagulants oraux, qui bloquent l'action de la vitamine K (antivitamine K, ou AVK) et dont l'efficacité est contrôlée par un dosage sanguin : l'INR (anciennement TP) ;
  • les anticoagulants injectables, dérivés de l'héparine, dont l'efficacité peut être contrôlée par le dosage sanguin de l'activité anti-Xa, le Temps de Howell (TH) ou le Temps de Cephalin Kaolin (TCK) suivant les produits utilisés. Un dosage régulier des plaquettes sanguines est nécessaire pendant toute la durée d'utilisation d'un dérivé de l'héparine.


anticonvulsivants
Médicament utilisé pour traiter les convulsions et les différentes formes d'épilepsie. Les anticonvulsivants sont classés en barbituriques (phénobarbital) et non barbituriques, qui sont les plus utilisés actuellement.


antidépresseur
Médicament qui agit contre la dépression. Certains antidépresseurs sont également utilisés pour combattre les troubles obsessionnels compulsifs, l'anxiété généralisée, certaines douleurs rebelles, l'énurésie, etc.
En fonction de leur mode d'action et de leurs effets indésirables, les antidépresseurs sont divisés en différentes familles : les antidépresseurs imipraminiques, les inhibiteurs de la recapture de la sérotonine, les IMAO (sélectifs ou non sélectifs). Enfin, d'autres antidépresseurs n'appartiennent à aucune de ces familles, car ils possèdent des propriétés originales.
Le mode d'action des antidépresseurs comporte deux aspects principaux : le soulagement de la souffrance morale et la lutte contre l'inhibition qui enlève toute volonté d'action au déprimé. Il arrive qu'un décalage survienne entre ces deux effets : la souffrance morale peut persister, alors que la capacité d'action réapparaît. Pendant cette courte période, le risque suicidaire présent chez certains déprimés peut être accru. Le médecin en tient compte dans sa prescription (association éventuelle à un tranquillisant) et celle-ci doit être impérativement respectée.


antiprotéases
Médicament antirétroviral qui agit en bloquant une enzyme (la protéase) nécessaire à la maturation du virus du sida (VIH) dans les cellules infectées. Ce type de médicament bloque l'une des étapes de la reproduction du virus, sans toutefois permettre son éradication. Il est le plus souvent utilisé en association avec d'autres antirétroviraux.


contraceptifs
Qui s'oppose à la conception : contraceptif oral (pilule), local (stérilet, spermicide, diaphragme et préservatif).


états dépressifs
État de souffrance morale associant une démotivation, une fatigue, un sentiment d'inutilité, d'autodépréciation. En l'absence de traitement, l'état dépressif risque d'évoluer vers une dépression grave avec sensation d'incurabilité, de culpabilité, et un risque suicidaire.


immunosuppresseurs

photosensibilisation
Sensibilité anormale de la peau à la lumière ou aux rayons ultraviolets, due à un médicament ou à une substance naturelle ou chimique.


réaction allergique
Réaction due à l'hypersensibilité de l'organisme à un médicament. Les réactions allergiques peuvent prendre des aspects très variés : urticaire, œdème de Quincke, eczéma, éruption de boutons rappelant la rougeole, etc. Le choc anaphylactique est une réaction allergique généralisée qui provoque un malaise par chute brutale de la tension artérielle.
