What is a gout attack, and how does it occur?

- Triggers: a sudden drop in serum uric acid levels (fasting, a heavy meal, alcohol, diuretics, or starting allopurinol) or trauma—causes the crystals to be released into the joint cavity
- Acute phase (6–12 hours): peak pain within 6–12 hours—often described as the worst pain ever experienced—intolerance to even the slightest touch (even the weight of a sheet)

What should you do during a gout attack?
Taking the right steps during an attack limits the duration and intensity of the episode—and prevents the situation from worsening.
- Seek urgent medical attention: colchicine (0.5 mg every 6–8 hours) is the standard treatment for an attack—effective if started within 12–24 hours of the onset of the attack
- Local cold therapy: Apply ice wrapped in a cloth for 15–20 minutes—this reduces local inflammation and provides temporary relief—never use heat (it worsens vasodilation and inflammation)
- Weight-bearing restriction: Avoid putting any weight on the joint—strict rest—elevate the limb if possible to reduce swelling
- Heavy hydration: 2–3 L of water per day — promotes renal excretion of uric acid and dilutes uric acid concentration
- Alcohol should be avoided: it increases uric acid production and reduces its excretion—absolutely contraindicated during a flare-up
- Do not start allopurinol during a flare-up—risk of prolonging or worsening the inflammatory episode
Vitamin C and the Prevention of Gout Attacks
Vitamin C has a well-documented mechanism of action on serum uric acid levels—its preventive role is recognized in the medical literature as a complement to long-term treatment.
- Vitamin C is uricosuric: it increases renal excretion of uric acid—a meta-analysis of 13 clinical trials shows a reduction in serum uric acid levels of 26 μmol/L per 500 mg/day of vitamin C
- Preventive dosage: 500–1,000 mg/day—to be taken in consultation with the treating physician for patients on allopurinol (as a supplement)
