What is biliary insufficiency, and how does it manifest?

Biliary insufficiency refers to insufficient production or secretion of bile by the liver—bile that is essential for the emulsification and absorption of dietary fats and fat-soluble vitamins (A, D, E, K). It can be functional (the liver produces less bile) or obstructive (bile is produced but blocked).   • Functional biliary insufficiency: reduced bile production by the liver—chronic liver diseases (cirrhosis, NASH, chronic hepatitis)—pale stools + fatigue + fat malabsorption
  • Cholestasis: obstruction of bile flow—intrahepatic (medications, primary biliary cirrhosis, pregnancy-related cholestasis) or extrahepatic (gallstones, pancreatic tumor, biliary stricture)
  • Postcholecystectomy (small reservoir syndrome): removal of the gallbladder → bile flows continuously without being stored → impaired fat digestion → fatty diarrhea + postprandial bloating — a common and underestimated condition
