What is aerophagia, and how does it manifest?

Aerophagia is a functional disorder characterized by the excessive swallowing of air—air that accumulates in the stomach and causes repeated belching or painful bloating. This should be distinguished from flatulence (gas produced by colonic fermentation)—in aerophagia, the air comes from outside the body, not from digestion.   • Main symptoms: frequent belching (> 3–4 times per meal), immediate postprandial epigastric bloating, a feeling of fullness from the very first bites, upper abdominal pain, and sometimes early satiety
  • Air swallowing: main mechanism — each swallow normally takes in 2–3 mL of air — in cases of aerophagia, this volume can reach 10–15 mL per swallow — the stomach can distend to hold up to 1 L of air
  • Main triggers: eating too quickly (swallowing air along with food), carbonated beverages, chewing gum, smoking (unconscious swallowing of air-laden saliva), drinking through a straw (inhaling air), stress (compulsive swallowing)

Behavioral and dietary strategies for aerophagia

  • Chew slowly: 20–30 chews per bite — reduces the amount of air swallowed with food — eat meals in at least 20–30 minutes — put down your utensils between each bite
  • Avoid carbonated beverages: each sip of a carbonated drink releases CO2 directly into the stomach—replace with still water or herbal teas
  • Stop chewing gum: Chewing without food = repeated swallowing of air + excessive salivation
  • Post-meal position: Stay seated or walk around gently—avoid lying down (which facilitates air regurgitation and GERD)

Natural and Medicinal Remedies for Aerophagia

  • Simethicone: anti-foaming agent — breaks down gas bubbles in the stomach → coalescence → easier elimination — no systemic absorption — safe for everyone, including infants and pregnant women — see our simethicone page
  • Activated charcoal: adsorbs stomach gas—provides rapid relief—should be taken at least 2 hours apart from medications and supplements
