What is simethicone, and how does it work to relieve gas?
Simethicone is an anti-foaming agent—the active ingredient in Polysilane® and many anti-bloating formulas—that acts locally in the stomach and intestines without ever being absorbed into the bloodstream. Its mechanism of action is purely physical. Our “Flat Stomach” line offers combination formulas that include simethicone.
- Anti-foaming mechanism: reduces the surface tension of gas bubbles—microbubbles coalesce into larger bubbles → easier to expel through belching (stomach) or via the anus (intestines)
- Purely local action: no systemic absorption—no effect on the brain, liver, or kidneys—no alteration of intestinal motility (≠ antispasmodics)
- Main indication: bloating, aerophagia, meteorism, gas colic—rapid relief (15–30 minutes after taking)
- Use before ultrasound or endoscopy: reduces gas in the digestive tract → improves the quality of ultrasound images — taken the day before and on the morning of the exam as prescribed
- Difference between simethicone and activated charcoal: simethicone = defoamer (acts on bubbles) — activated charcoal = adsorbent (absorbs gases and odor-causing molecules, but may also adsorb medications → take at least 2 hours apart)
Dosage forms and dosage based on patient profile
- Tablets and capsules: adults — 80–125 mg per dose — to be taken after meals and/or at bedtime — suitable for recurrent postprandial bloating
- Oral suspension (drops): infants and young children — administered before or after a bottle or meal — liquid formula facilitates precise dosing based on weight

Simethicone for infants: colic and tolerance
- Infant colic: unexplained crying for > 3 hours/day in a baby < 3 months old, especially in the evening — simethicone reduces gas buildup in the baby’s immature intestines
- Optimal safety profile: does not enter the bloodstream → no systemic effects on the baby — no significant allergies reported — no dependence or addiction
- Pediatric formulas without sugar or artificial colors: preferred for infants and young children — check the ingredients before purchasing
- Clinical data on colic: meta-analyses show a moderate benefit compared to placebo — to be combined with babywearing, abdominal massage, and prone positioning after meals (under supervision)
- Natural alternatives for colic: fennel (diluted herbal tea for infants > 6 months), gentle circular abdominal massage
Contraindications, precautions, and combination with other active ingredients
- Absolute contraindications: intestinal obstruction (never use anti-flatulent agents if bowel movement is completely stopped), allergy to silicones (rare), unexplained and severe abdominal pain without a diagnosis
- Pregnancy and breastfeeding: not absorbed — used without specific restrictions — inform your doctor
- Combination with activated charcoal: possible and synergistic—simethicone breaks down gas bubbles, while charcoal adsorbs odor-causing molecules—take at least 2 hours apart from any other medication
