Our advice and opinions from pharmacy experts

Relaxation exercise: Abdominal breathing.

The side effects of smoking cessation are very numerous: stress, mood disorders, physiological disturbances, intestinal problems, etc.
These side effects appear from the start of smoking cessation and represent the main cause of relapse. These different symptoms correspond to our body's response to the lack of nicotine.
To avoid getting into this vicious vicious circle, we offer you a little exercise in breathing applicable on a daily basis.
The latter will allow you to relax, unwind and calm down ...

The deep breathing located at the ventral level helps to remain relaxed because it helps to quickly decrease anxiety. By learning to breathe in this way, you will be able to control your emotions and regain calm in moments of anxiety and nervousness. Easy and quick to practice, it is therefore an excellent ally for stopping smoking.

Breathing from the lower abdomen is a habit not easy at first, but with a little training, you can easily practice it when and where you want. Here are some tips for learning how to breathe well. Train yourself !

  • Make yourself comfortable . Possibly loosen your belt, unplug your phone for a few moments, if you are cold, cover up. In short, make yourself comfortable.
  • Inhale . To help you become aware of this breathing, place your hands on your stomach. Start breathing slowly, closing your mouth and not shrugging your shoulders. Take air through your nose, you will feel your belly come out, swell gently….
  • Mark a break time. When inspiration is complete, hold the air for 2 to 4 seconds and focus on what's going on inside you.
  • Exhale . Then release very gently, and through the nose, first of all without forcing, the air that you have retained during these few seconds, then you accompany the exhalation by returning the belly.

Do not underestimate the effectiveness of this breathing! In its very simple aspects, it is a real way to release tensions, especially in the event of an irrepressible urge to smoke. Practice it as many times as you want and especially in times of stress or cravings.

COUNCIL / HEALTH EDUCATION

As soon as you stop smoking:

· You will no longer absorb the irritating and toxic substances of cigarettes,

· You will see some improvements: progressive disappearance of headaches, cough, irritation of the throat, shortness of breath,

· You will often have the satisfaction of getting better sleep, flavor of food, smells and improving your physical performance,

· You will reduce your risk of developing lung cancer or cardiovascular disease.

It's never too late to quit even if you have smoked a long time and a lot.

What is smoking addiction?

Tobacco addiction is twofold:

· Psychological dependence:
Smoking a cigarette entails a ritual of automatic and repetitive gestures associated with particular circumstances (pleasure, stress response, intellectual stimulation, support for morale, need for the gesture of conviviality). This is the psychological and behavioral dependence.

· Physical dependence:
The body's need for nicotine is due to the effects of this substance on the nervous system. It's about physical dependence. The non-satisfaction of this need leads to a feeling of lack (see description in the chapter "How to overcome your physical dependence?").

Nicotine substitutes aim to treat this dependence.

How to overcome your psychological dependence?

· Complete smoking cessation is necessary to increase your chances of success, but more:

o Choose a day J in the period that seems most favorable to you.

o Throw away your last pack of cigarettes, your lighter and put away your ashtrays.

o In a couple of smokers, it is preferable that the arrest attempt be made at the same time by both spouses.

· Tell your family and coworkers that you are quitting. Ask them to respect your decision (do not smoke near you, do not offer cigarettes).

· Prepare to deal with the desire to smoke. It can occur abruptly and intensely especially during the first few weeks after stopping smoking, but does not last more than 3 or 4 minutes. You must anticipate what you will do in this case and act very quickly not to "crack" like for example:

o drink a big glass of water,

o to chew gum,

o count to 100,

o to make some deep breaths,

o to leave the room,

o change of activity ...

· Avoid situations associated with taking cigarettes (coffee, aperitif, ...), avoid temptations (settle with non-smokers).

· Find compensation for cigarettes (leisure, sport, cultural activity ...).

How to overcome your physical dependence?

If you are addicted to nicotine, the success of your approach may require the use of nicotine substitutes.

To assess your level of dependence, the Fagerström test is offered.

TEST OF FAGERSTROM

How long after you wake up do you smoke your first cigarette?

· within 5 minutes: 3

· from 6 to 30 minutes: 2

· from 31 to 60 minutes: 1

· after 60 minutes: 0

Do you find it difficult not to smoke in places where it is forbidden?

· yes: 1

· No: 0

Which cigarette of the day would it be the hardest to give up?

· the first: 1

· another: 0

How many cigarettes do you smoke a day?

· 10 or less: 0

· 11 to 20: 1

· 21 to 30: 2

· 31 or more: 3

Do you smoke at a faster pace in the morning than in the afternoon?

· yes: 1

· No: 0

Do you smoke when you are so sick that you have to stay in bed most of the day?

· yes: 1

· No: 0

TOTAL POINTS

· score from 0 to 2: you are not addicted to nicotine.
You can stop smoking without the need for a nicotine replacement.
However, if you are dreading this decision, consult your pharmacist or doctor.

· score 3 to 4: you are weakly dependent on nicotine.

· score 5 to 6: you are moderately addicted to nicotine.
The use of nicotine substitutes will increase your chances of success.
Take advice from your pharmacist or doctor to help you choose the most appropriate treatment for you.

· score 7 to 10: you are strongly or very strongly dependent on nicotine.
The use of nicotine replacement is recommended to help you overcome this addiction to nicotine. This treatment must be used in sufficient and appropriate doses.
Take advice from your pharmacist or doctor, possibly as part of a specialized quit smoking consultation.

Withdrawal disorders may persist for a long time and the duration of treatment should be sufficient, usually one to three months.

If you feel despite the treatment, a feeling of persistent lack such as:

· craving to smoke,

· irritability, sleep disorders,

· agitation or impatience,

· difficulty concentrating,

Take advice from your doctor or pharmacist as dose adjustment may be necessary.

How to help you?

· The support of health professionals
The accompaniment, the advice, the follow-up that can bring you your doctor or your pharmacist will be precious during the first six months of your stop of the tobacco. You can also use a specialized smoking cessation counseling, especially if you have already failed.

· Psychological support
Quitting smoking requires a supportive family and social environment. If you are concerned that the difficulties are too great, psychological support or temporary treatment can help you.

· Dietetics
In the months after stopping smoking, you may notice moderate weight gain.
If you have concerns in this area, you can contact a health professional who will advise you.

You started smoking again, why?

Consider this relapse as an accident of course, as a normal event that allows you to learn: relapse is not a failure.

Analyze why you started smoking again.

· Because of physical dependence
This may have been poorly taken into account or may reappear after several months, for example, prolonged stays in smoky atmospheres.

· Because of weight gain deemed excessive.
It may be necessary to change your eating habits a little before your next quit attempt.

· Because of a decrease in motivation, because of a painful event or a difficult period, because of psychological difficulties appeared at the stop of the tobacco (depression, anxiety ...), because of a situation very tempting. Understand the reason for this relapse to prevent it if possible during the next attempt.

In any case, do not hesitate to seek advice from a health professional or to resort to a specialized consultation of help with smoking cessation.

If you have started smoking again, do not be discouraged, experience shows that your chances of success increase after one or more stopping attempts.

Nicotine 1 mg 2 mg

INDICATIONS (summary)
This medication helps smokers to detoxify themselves from tobacco. The tablet brings into the blood, through saliva, the nicotine, the main active ingredient of tobacco, responsible for addiction.
CONTRAINDICATIONS (summary)
This medicine should not be used in the following cases:
ATTENTION (summary)
Stopping smoking is imperative before starting treatment.
Precautions are necessary in case of serious heart disease, hypertension , stomach or duodenal ulcer , hyperthyroidism , pheochromocytoma , diabetes , renal or hepatic insufficiency, or arteritis of the lower limbs. In these situations, prior medical consultation is essential before using this medication; stopping smoking without substitution treatment may be necessary.
Do not use other medicines containing nicotine without medical advice, especially transdermal patches .
Do not let this medicine within the reach of children.
If, after 10 days of treatment, you start smoking again: consult your doctor to adopt a new smoking cessation strategy.
DRUG INTERACTIONS (summary)
The products contained in tobacco smoke can accelerate the elimination rate of certain drugs, theophylline in particular. When stopping smoking, these drugs may have an increased effect and signs of overdose may occur. Contact your doctor if there is any doubt.
Absorbing coffee or soda within 15 minutes of taking the tablet to suck may decrease the absorption of nicotine and therefore the effect of the drug.
PREGNANCY and BREASTFEEDING (summary)
Pregnancy :
Smoking in pregnant women can cause fetal growth retardation. Stopping smoking is therefore always desirable. In women highly dependent on nicotine, the use of this drug may be helpful, but the effects of nicotine alone on the fetus are poorly understood. This drug has the advantage of not exposing the fetus to other toxins found in tobacco smoke. In summary: if the ideal is not to smoke and not expose the fetus to nicotine intake, the use of this drug is a lesser evil compared to tobacco.

Breastfeeding:
Nicotine passes into breast milk, breastfeeding is not recommended during treatment.
If breastfeeding is nevertheless chosen, take the tablets preferably after the head.
DIRECTIONS AND DOSAGE (summary)
The choice of dosage depends on the initial dependence on nicotine. 2 mg tablets are suitable for highly or highly addicted smokers.
In practice, it is useful to know the signs of under or overdose to adjust the dosage after a few days of treatment:
  • Overdose results in the smoker's over-smoking status, which mainly results in nausea and headaches. Visual or hearing disorders, tremor, vertigo are also possible;
  • under-dosing causes withdrawal symptoms : irritability, nervousness, stomach cramps (a kind of painful hunger), difficulty concentrating.
As soon as the desire to smoke is overcome, gradually reduce the number of tablets, until complete cessation.
In order to allow slow and regular absorption of nicotine by the blood vessels of the tongue, suck the tablet until the taste becomes strong, then place it against the cheek while the taste fades in the mouth . Then repeat the operation. After 30 minutes, the dissolution of the tablet must be complete.

Usual dosage:

  • Adults over the age of 18: suck one tablet each time the need to smoke is felt, which usually corresponds to 8 to 12 tablets a day. Do not exceed 30 1 mg tablets or 15 2 mg tablets daily.
Do not exceed 6 months of continuous treatment.
TIPS (summary)
This medication can be used by smokers without medical consultation. It is available without a prescription and may be in direct access in some pharmacies. Nevertheless, do not hesitate to have your choice validated by your pharmacist. His advice is precious. He can help you determine the best time to quit and explain the causes of failure.
Nicotine taken by way oral replacing the cigarette has the same cardiovascular harmful than that in tobacco. Only the risk of lung cancer disappears. When the smoking cessation is motivated by urgent medical reasons: recent infarction , arteritis , gastritis or ulcer , it is better to stop the absorption of nicotine in all its forms.
Like all anti-smoking methods used to help smokers, this medication can only be effective when the will to quit is personal and comes with a strong motivation. it is this motivation which will make it possible to fight against the desire to smoke, without being forced to make permanent appeal to the will that dulls with the time. A smoker's cessation of tobacco smoking supposes that he considers tobacco to be a poison, an unacceptable dependence. He must be convinced that he will never smoke again, from the beginning of weaning. Every day without tobacco must be lived as a liberation, and not as a nightmare. The smoker who is in this state of mind is a good candidate for permanent weaning.
The cessation of smoking, which is a real drug, causes many problems: weight gain is common, disorders of the concentration can last several weeks. A slight depressive state is possible. Weight gain must be fought by a diet, and if possible by a sports program that has the advantage of reconciling the smoker with his body.
Do not hesitate to consult your doctor in case of difficulty; his advice and experience will be invaluable in fighting relapse.
ADVERSE POSSIBLE EFFECTS (summary)
Irritation of the mouth or throat.
Nausea, heartburn, indigestion, hiccups, headache, dizziness, rash , palpitations .
Rarely: heart rhythm disorders , allergic reaction .

General term for diseases resulting in the emission of abundant urine and intense thirst. Employed alone, the word diabetes generally concerns diabetes mellitus, which corresponds to a disruption of the body's sugar regulation by insulin. It results in the increase of sugar (glucose) in the blood and by its possible presence in the urine. There are two types of diabetes mellitus: type 1 diabetes, or insulin-dependent diabetes, which requires treatment with insulin injection, and type 2 diabetes, or non-insulin-dependent diabetes mellitus, which may initially be treated with oral antidiabetic drugs.

transdermal patches
System, sometimes called patch or patch, allowing the absorption of a drug through the skin: it ensures the diffusion of the active substance to the blood vessels of the dermis. The substance then circulates in the blood and can act as if it had been swallowed or injected. This device avoids the effect of first pass liver.

hypertension
Excess pressure (tension) of the blood in the arteries. This pressure is provided by the heart and is expressed by two numbers. It is maximal during the contraction of the heart, or systole (first digit), and minimum during the cardiac rest, or diastole (second digit). Hypertension is characterized by a maximal (systolic) pressure greater than or equal to 16, or a minimum (diastolic) pressure greater than or equal to 9.5. Treatment is initiated when these numbers are exceeded or for lower voltages in some people with cardiovascular risk factors: heredity, smoking, diabetes, excess cholesterol. Permanent antihypertensive therapy can reduce the damage to the heart and blood vessels caused by high blood pressure.

hyperthyroidism
Excess thyroid hormones including heart acceleration, poor heat tolerance, weight loss and chronic fatigue.

infarct
Destruction of part of the heart muscle (myocardium), deprived of blood by obstruction of its arteries.

Inability of the liver to perform its function, which is essentially the elimination of certain wastes, but also the synthesis of many biological substances essential to the body: albumin, cholesterol and coagulation factors (vitamin K, etc.).

renal failure
Inability of the kidneys to dispose of waste or drugs. Advanced renal failure does not necessarily result in a decrease in the amount of urine eliminated. Only a blood test and the determination of creatinine can reveal this disease.

palpitations
Abnormal perception of irregular heartbeat.

phenylketonuria
Hereditary disease that is characterized by the absence of an enzyme and leads to the accumulation in the blood of a toxic product. Its screening is systematic at birth. Treatment is based on a specific diet during infancy.

allergic reaction
Reaction due to the hypersensitivity of the body to a drug. Allergic reactions can be very varied: urticaria, angioedema, eczema, rash of measles-like pimples, etc. Anaphylactic shock is a generalized allergic reaction that causes discomfort by sudden drop in blood pressure.

overdose
Excessive intake of a drug may lead to an increase in the intensity of the adverse effects, or even to the occurrence of particular adverse effects.
This overdose can result from an accidental intoxication, or voluntary for the purpose of suicide: it is then advisable to consult the poison control center of your region (list in appendix of the work). But most often, the overdose is the consequence of an error in the understanding of the prescription, or the search for an increase of the effectiveness by a surpassing the recommended dosage. Finally, untimely self-medication can lead to the excessive absorption of the same substance contained in different drugs. Some drugs are particularly exposed to this risk, because they are considered (wrongly) as harmless: vitamins A and D, aspirin, etc. Stopping or decreasing drug intake can eliminate the problems associated with overdose.

symptoms
Disorder felt by a person with an illness. The same symptom can translate different diseases and the same disease does not necessarily give the same symptoms in all patients.

heart rhythm disorders
Serious or benign abnormality of the frequency of contractions of the heart. Extrasystole is a contraction occurring just before or after a normal contraction, often perceived as a slight shock in the chest. Fibrillation is an irregular and disordered contraction. Other disorders include torsades de pointes, Wolf-Parkinson-White syndrome, Bouveret's disease, tachysystole, flutter and atrioventricular block, etc.
