Dry skin (or xerosis) refers to skin that chronically lacks water and/or lipids in its stratum corneum. It differs from simple constitutional dry skin in its severity: visible flakes, constant tightness, itching, roughness, and sometimes painful cracks. Xerosis can be primary (atopic dermatitis, ichthyosis, psoriasis) or secondary to extrinsic factors (central heating, hard water, detergent soaps, medications such as retinoids or diuretics). The primary mechanism is increased transepidermal water loss (TEWL) due to a deficiency in ceramides and natural moisturizing factors (NMF). Glycerin (glycerol) is the first humectant to restore—it draws water from the deeper layers and rapidly reduces TEWL.
Skincare products for dry skin fall into three complementary categories:
Dexpanthenol (provitamin B5, Bepanthensensicalm Anti-Itch Cream) accelerates the regeneration of the skin barrier and reduces itching caused by dryness.
Choosing the right cleanser is just as important as choosing a moisturizer—ordinary soap with an alkaline pH (9–10) breaks down the lipid barrier with every wash. Niacinamide (vitamin B3) acts on several mechanisms simultaneously: it stimulates ceramide synthesis, increases keratin production, reduces TEWL, and has a mild anti-inflammatory effect. Most cases of dry skin respond well to emollient treatments within 4 to 8 weeks. Certain signs warrant a consultation: persistence despite a rigorous 4-week emollient regimen, infected cracks or lichenified patches, intense nighttime itching that disrupts sleep, or xerosis associated with medication use. Dry skin in older adults or people with diabetes requires specific management.
