2. WHAT DO I HAVE TO KNOW BEFORE USING NICORETTESKIN 25 mg/16 hours, transdermal device?
Never use NICORETTESKIN 25 mg/16 hours, transdermal device:
- if you are allergic to nicotine or any of the other ingredients of this product, as listed in section 6.
- if you do not smoke or if you are an occasional smoker,
- if you suffer from a skin condition that may interfere with the use of a transdermal system.

Warnings and precautions

In order to successfully stop smoking, it is essential to quit completely.
Read the health education advice carefully.
Certain diseases require medical examination and follow-up before using this medication.
It is therefore essential to consult your doctor before using this medicine if you have :
- a recent history (< 3 months) of heart attack or stroke,
- chest pain (unstable angina pectoris), or stable angina pectoris,
- a heart problem affecting your heart rate or rhythm,
- high blood pressure not controlled by medication,
- a history of allergic reactions manifested by swelling of the lips, face and throat (angioedema) or an itchy rash (urticaria). The use of NRT may trigger this type of reaction,
- severe or moderate hepatic insufficiency,
- severe renal insufficiency
- diabetes,
- hyperthyroidism,
- a tumor of the adrenal gland (pheochromocytoma),
- an evolving stomach or duodenal ulcer,
- chronic throat disease,
- asthma,
- a history of seizures or convulsions.
Keep the transdermal device out of the reach of children before and after use.
The therapeutic dose intended for adults could cause serious or even fatal intoxication in children.

Pregnancy

If you are unable to stop smoking without nicotine replacement therapy, NICORETTESKIN may be used in consultation with the healthcare professional who is monitoring your pregnancy, your family doctor, or a doctor at a center specializing in helping people who want to stop smoking.
Breast-feeding
If you are a heavy smoker and do not intend to stop smoking, you should not breast-feed your child, but use artificial milk. The use of this medication is not recommended while breast-feeding, as nicotine passes into breast milk and may affect your child.
However, if you wish to stop smoking and breastfeed your child, seek the advice of your doctor or a doctor at a center specializing in smoking cessation.
Driving and using machines
NICORETTESKIN 25 mg/16 hours, transdermal device has no or negligible effect on the ability to drive and use machines.
3. HOW DO I USE NICORETTESKIN 25 mg/16 hours, transdermal device?
Always use NICORETTESKIN exactly as instructed in this leaflet or as directed by your doctor or pharmacist. Check with your doctor or pharmacist if in doubt.
For adults and children over 15 years of age.
Dosage
Three transdermal device sizes are available: 9; 13.5; 22.5 cm2, corresponding to 3 different dosages: 10 mg/16 hours, 15 mg/16 hours, 25 mg/16 hours.
The transdermal device should be applied to a healthy part of the skin in the morning on rising, and removed in the evening at bedtime, i.e. around 16 h later. The treatment mimics nicotine fluctuations during the day in smokers, without the administration of nicotine at night. Daytime administration of nicotine by the transdermal device does not cause the sleep disturbances seen with nighttime nicotine administration.
- It is recommended for highly dependent smokers (Fagerström test score ≥ 7) to start with the transdermal device 25 mg/16 hours. A transdermal device will be applied daily in the morning and removed in the evening, for 8 weeks.
- Dose reduction is then initiated. A 15 mg/16 hours transdermal device is used daily for 2 weeks, followed by a 10 mg/16 hours transdermal device, also for 2 weeks.
- It is recommended for moderately dependent smokers (Fagerström test scores 5 and 6) to start with step 2, using the 15 mg/16 hour transdermal device.
o If smoking cessation is achieved in the first week with the 15 mg/16 hours dosage, a transdermal device will be applied daily in the morning and removed in the evening, for 8 weeks, then a 10 mg/16 hours transdermal device will be applied for 4 weeks.
o If, after 1-7 days' use of the 15 mg/16 hours transdermal device, signs of craving persist, it is recommended that a 25 mg/16 hours transdermal device be used, and that treatment be continued according to the schedule for heavily dependent patients (see "heavily dependent smokers").
- It is recommended that lightly dependent smokers (Fagerström test scores 3 and 4) are recommended to start with step 2, using the 15 mg/16 hour transdermal device, applied daily for 8 weeks, and to continue with the 10 mg/16 hour transdermal device, applied for 4 weeks.

Overall treatment should last at least 3 months.
However, the duration of treatment may vary according to individual response.
It is recommended not to use transdermal devices beyond 6 months. However, some "ex-smokers" may require a longer duration of treatment to avoid relapse.
If, during your treatment, you continue to experience cravings to smoke, or if your treatment has failed, ASK YOUR DOCTOR OR PHARMACIST FOR ADVICE.
Your doctor or pharmacist may advise you to combine NICORETTESKIN transdermal devices with oral substitutes (NICORETTESKIN chewing gum, NICORETTESKIN tablets).NICORETTE 2 mg chewing gum, NICORETTE 10 mg inhaler, NICORETTE 2 mg chewable tablets, NICORETTE MICROTAB 2 mg sublingual tablets or NICORETTESPRAY 1 mg/dose oral spray solution).
When NICORETTESKIN is used in combination with an oral substitute, the dosage is the same as when these drugs are used alone as monotherapy. For the dosage of the oral form, consult the package insert for the chosen medication.
In case of combination, use only one oral form at a time over a 24 hour period.
Directions for use
- Wash your hands before applying the transdermal device.
- Open the sachet with a pair of scissors and remove the transdermal device.
- Immediately after opening, apply the transdermal device to a dry, hairless area of skin (hip, upper arm or chest).
- Remove as much of the aluminium foil as possible. As far as possible, avoid touching the sticky surface of the transdermal device with your fingers.
- Apply the sticky part with your fingers and remove the remaining aluminium foil.
- Press the transdermal device firmly onto the skin with your fingers.
- Rub the edges firmly with your fingers to ensure that the transdermal device sticks properly.
- The transdermal device should be applied in the morning and removed at bedtime, around 16 hours later.
- The application site should be changed every day, avoiding if possible replacing the transdermal device in the same place for a few days.
- It is essential to fold the transdermal device onto itself after use, before disposing of it in a safe place.
- All transdermal devices, whether used or not, should be kept out of the reach of children.
- During handling, avoid contact with eyes and nose, and wash hands after application.
- Do not cut the transdermal device.
- In the event of prolonged sea or pool bathing, you have 2 options :
o Before bathing, remove the transdermal device and immediately replace it in its holder. It can then be re-adhered to dry skin.
o Cover the transdermal device with a waterproof adhesive bandage for the duration of the bath.
- In the event of a short shower, the transdermal device can be held in place by avoiding directing the shower jet onto it.

4. WHAT ARE THE POSSIBLE SIDE EFFECTS?
Like all medicines, this drug may cause undesirable effects, but they do not occur systematically in everyone.
Very common: affects more than 10 out of 100 users
- itching (pruritus).
Common: affects 1 to 10 users out of 100
- rash and urticaria,
- nausea and vomiting,
- digestive disorders,
- skin redness,
- headache,
- dizziness.
Uncommon: affects 1 to 10 users in 1000
- palpitations (unusual sensation in your heartbeat), or rapid heartbeat (tachycardia),
- abnormal dreams,
- excessive sweating, hot flushes,
- fatigue, malaise, chest pain/discomfort, difficulty breathing (dyspnea),
- application site reaction,
- allergic reactions,
- hypertension,
- muscle pain,
- tingling sensation that may spread around the application site.
Very rare: affects less than 1 in 10,000 users.
- Heart rhythm disorders.
Undetermined frequency: cannot be estimated on the basis of available data.
- pain/discomfort in the abdomen,
- allergic reaction including swelling of the face and mouth (angioedema),
- convulsions.
Dizziness, headaches, sleep disturbances, mouth ulcers may be related to smoking cessation and not to your treatment.

Keep out of sight and reach of children.

What is tobacco dependence?
Tobacco addiction is twofold:
- Psychological dependence :

- Physical dependence :

How to overcome your psychological dependence?
- Stopping smoking altogether is necessary to increase your chances of success:

o In the case of a smoking couple, it's preferable for both partners to quit at the same time.

Ce médicament aide les fumeurs à se désintoxiquer du tabac. Le dispositif transdermique (timbre) contient de la nicotine, principale substance active du tabac ; celle-ci traverse la peau, gagne le sang et agit sur le système nerveux, permettant ainsi au fumeur de ne pas ressentir les symptômes dus à l'arrêt du tabac.

Dans l'optique d'une désintoxication définitive, l'arrêt du tabac est une condition nécessaire au succès du traitement.
N'utilisez pas d'autres médicaments contenant de la nicotine, notamment les gommes à mâcher (chewing-gums) sans avis médical.
Des précautions sont nécessaires en cas d'ulcère de l'estomac ou du duodénum, d'insuffisance rénale ou d'insuffisance hépatique : dans ces situations, une consultation médicale préalable est indispensable avant d'utiliser ce médicament et un arrêt du tabac sans traitement de substitution peut être nécessaire.
Ne laissez pas le timbre neuf ou usagé à la portée des enfants : une application intempestive peut être à l'origine d'accidents graves. Pliez le timbre avant de le jeter.
Les produits contenus dans la fumée du tabac peuvent accélérer la vitesse d'élimination de certains médicaments, la théophylline notamment. Lors de l'arrêt du tabagisme, ces médicaments peuvent voir leur effet renforcé et des signes de surdosage peuvent apparaître. Contactez votre médecin au moindre doute.
Le tabagisme chez la femme enceinte peut être à l'origine d'un retard de croissance du fœtus. L'arrêt du tabac est donc toujours souhaitable. Chez les femmes fortement dépendantes à la nicotine, l'usage de ce médicament peut être une aide, mais les effets de la nicotine seule sur le fœtus sont mal connus. Ce médicament a l'avantage de ne pas exposer le fœtus aux autres toxiques présents dans la fumée du tabac. En résumé : si l'idéal est de ne pas fumer et de ne pas exposer le fœtus à des apports de nicotine, l'usage de ce médicament est un moindre mal par rapport au tabac.
La nicotine passe dans le lait maternel, l'allaitement est déconseillé pendant le traitement.
Il existe 3 dosages correspondant à 3 surfaces de timbre différentes : la quantité de nicotine qui passe dans le sang est proportionnelle à la surface du dispositif transdermique.
Le dosage le plus fort est destiné aux grands fumeurs ou aux fumeurs de cigarettes riches en nicotine ; une forte dépendance à la nicotine peut également conduire des fumeurs modérés à préférer le dosage fort.
Une fois le sevrage obtenu, il convient de passer à un timbre moins dosé après une période de 8 semaines, puis à l'arrêt après l'utilisation du dosage le plus faible.
Mettre le timbre sur une région sèche, pauvre en poils, et dépourvue de lésions cutanées. Libérer la surface adhésive et appliquer soigneusement le timbre en effectuant une pression sur toute sa surface. Changer d'emplacement tous les jours.
Pensez à vous laver les mains après l'application et évitez tout contact avec les yeux ou le nez.
Il est recommandé de ne pas dépasser 12 mois de traitement continu.
Ce médicament peut être utilisé seul ou en association avec les gommes à 2 mg ou l'inhaleur NICORETTE. Dans ce cas, conformez-vous aux recommandations de votre médecin.
La nicotine prise par voie cutanée en remplacement de la cigarette présente la même nocivité cardiovasculaire que celle qui est contenue dans le tabac. Seul le risque de cancer du poumon disparaît. Lorsque l'arrêt du tabac est motivé par des raisons médicales urgentes : infarctus récent, artérite, gastrite ou ulcère, il est préférable d'arrêter l'absorption de nicotine sous toutes ses formes.
Comme toutes les méthodes antitabac utilisées pour aider les fumeurs, ce médicament ne peut être efficace que lorsque la volonté d'arrêter de fumer est personnelle et s'accompagne d'une forte motivation. C'est cette motivation qui permettra de lutter contre l'envie de fumer, sans être forcé de faire en permanence appel à la volonté qui s'émousse avec le temps. Un arrêt du tabac durable chez le fumeur suppose que celui-ci considère le tabac comme un poison, une dépendance inacceptable. Il doit se faire à l'idée qu'il ne fumera plus jamais, dès le début du sevrage. Chaque jour sans tabac doit être vécu comme une libération, et non comme un calvaire. Le fumeur qui est dans cet état d'esprit est un bon candidat au sevrage définitif.
L'arrêt du tabac, qui est une véritable drogue, induit de nombreux problèmes : une prise de poids est fréquente, des troubles de la concentration peuvent durer plusieurs semaines. Un état dépressif léger est possible. La prise de poids doit être combattue par un régime, et si possible par un programme sportif qui a l'avantage de réconcilier le fumeur avec son corps. N'hésitez pas à consulter votre médecin en cas de difficulté ; ses conseils et son expérience seront précieux pour lutter contre les rechutes.
Très fréquents (plus de 10 % des personnes) : rougeur et démangeaisons au niveau de l'application pouvant nécessiter l'arrêt du traitement en cas de réaction importante.
Fréquents (1 à 10 % des personnes) : maux de tête, étourdissements, nausées, vomissements, troubles digestifs, éruption cutanée, urticaire.
Peu fréquents (0,1 à 1 % des personnes) : transpiration excessive, bouffées de chaleur, palpitations, tachycardie, fourmillements, rêves anormaux, douleurs musculaires, fatigue, essoufflement.
Très rares (moins de 0,01 % des personnes) : troubles du rythme cardiaque.
Fréquence indéterminée : œdème de Quincke.
Maladie des artères favorisée par le tabagisme et le plus souvent localisée aux jambes. Elle se caractérise par un épaississement des parois artérielles qui gêne la circulation du sang et entraîne des douleurs au cours de la marche (claudication intermittente). L'artérite des membres inférieurs augmente le risque de survenue d'infarctus du myocarde et nécessite un traitement médicamenteux. À un stade évolué, elle peut être traitée chirurgicalement.
Synonyme : artériopathie oblitérante des membres inférieurs.

Maladie due au développement anarchique et incontrôlable de certaines cellules de l'organisme. Ces cellules anormales, capables de se multiplier indéfiniment, peuvent envahir des organes ou des vaisseaux sanguins et lymphatiques, formant à distance de nouvelles tumeurs, appelées métastases. On classe les cancers en tumeurs solides (qui se développent à partir d'un organe visible) et en tumeurs liquidiennes, qui proviennent des cellules de la moelle osseuse (leucémies notamment).

Besoin psychique ou physique entraîné par la prise de certaines substances naturelles ou médicamenteuses. Cet état peut s'accompagner, à l'arrêt des prises, de symptômes variés, regroupés sous le terme de syndrome de sevrage.
Système, parfois appelé timbre ou patch, permettant l'absorption d'un médicament au travers de la peau : il assure la diffusion de la substance active vers les vaisseaux sanguins du derme. La substance circule ensuite dans le sang et peut agir comme si elle avait été avalée ou injectée. Ce dispositif permet d'éviter l'effet de premier passage hépatique.
Partie de l'intestin dans laquelle pénètrent les aliments issus de l'estomac.
Apparition de boutons ou de plaques sur la peau. Ces lésions peuvent être dues à un aliment, à un médicament, et traduire une allergie ou un effet toxique. De nombreux virus peuvent également provoquer des éruptions de boutons : celles de la rubéole, de la roséole et de la rougeole sont les plus connues.
État de souffrance morale associant une démotivation, une fatigue, un sentiment d'inutilité, d'autodépréciation. En l'absence de traitement, l'état dépressif risque d'évoluer vers une dépression grave avec sensation d'incurabilité, de culpabilité, et un risque suicidaire.

La fréquence d'un effet indésirable est décrite dans les documents officiels de la manière suivante :
Inflammation de l'estomac favorisée par le stress, le tabac, l'alcool et certains médicaments tels que l'aspirine ou les anti-inflammatoires.
Incapacité du foie à remplir sa fonction, qui est essentiellement l'élimination de certains déchets, mais également la synthèse de nombreuses substances biologiques indispensables à l'organisme : albumine, cholestérol et facteurs de la coagulation (vitamine K, etc.).
Incapacité des reins à éliminer les déchets ou les substances médicamenteuses. Une insuffisance rénale avancée ne se traduit pas forcément par une diminution de la quantité d'urine éliminée. Seuls une prise de sang et le dosage de la créatinine peuvent révéler cette maladie.
Perception anormale de battements cardiaques irréguliers.

Réaction allergique touchant généralement le visage. L'œdème de Quincke se traduit par un gonflement parfois spectaculaire. Les paupières sont souvent les premières touchées. Dans les rares cas où l'œdème touche la gorge, des troubles respiratoires peuvent survenir et un traitement urgent est nécessaire.
La prise en quantité excessive d'un médicament expose à une augmentation de l'intensité des effets indésirables, voire à l'apparition d'effets indésirables particuliers.
Ce surdosage peut résulter d'une intoxication accidentelle, ou volontaire dans un but de suicide : il convient alors de consulter le centre antipoison de votre région (liste en annexe de l'ouvrage). Mais le plus souvent, le surdosage est la conséquence d'une erreur dans la compréhension de l'ordonnance, ou de la recherche d'une augmentation de l'efficacité par un dépassement de la posologie préconisée. Enfin, une automédication intempestive peut conduire à l'absorption en quantité excessive d'une même substance contenue dans des médicaments différents. Certains médicaments exposent plus particulièrement à ce risque, car ils sont considérés (à tort) comme anodins : vitamines A et D, aspirine, etc. L'arrêt ou la diminution des prises médicamenteuses permettent de faire disparaître les troubles liés à un surdosage.
Trouble ressenti par une personne atteinte d'une maladie. Un même symptôme peut traduire des maladies différentes et une même maladie ne donne pas forcément les mêmes symptômes chez tous les malades.
Accélération du rythme des battements du cœur. Elle peut être due à une fièvre, à une hyperthyroïdie, à une maladie cardiaque, à l'action de certains médicaments, etc.

Anomalie grave ou bénigne de la fréquence des contractions du cœur. L'extrasystole est une contraction survenant juste avant ou après une contraction normale, souvent perçue comme un léger choc dans la poitrine. La fibrillation est une contraction irrégulière et désordonnée. D'autres troubles existent : torsades de pointes, syndrome de Wolf-Parkinson-White, maladie de Bouveret, tachysystolie, flutter et bloc auriculoventriculaire, etc.

Ensemble de symptômes traduisant une irritation ou un mauvais fonctionnement du tube digestif. Un ou plusieurs troubles peuvent être présents : nausées, vomissements, aérophagie, douleurs abdominales, brûlures d'estomac, ballonnements, flatulences, diarrhée ou constipation, etc. Les antibiotiques peuvent favoriser les candidoses, souvent responsables de troubles digestifs.
Lésion en creux de la peau, des muqueuses ou de la cornée.
Éruption de boutons sur la peau, dont l'origine est le plus souvent allergique. Les boutons ressemblent à des piqûres d'orties et leur couleur varie du rose pâle au rouge. L'urticaire peut apparaître ou disparaître en quelques minutes, et provoque d'intenses démangeaisons ; elle peut être localisée ou généralisée à la totalité de la surface cutanée.
Symptôme qui peut désigner une impression de perte d'équilibre (sens commun) ou, plus strictement, une sensation de rotation sur soi-même ou de l'environnement (sens médical).
